# Worksheet instructions

These files contain a blank template and synthetic worked example. They are planning records, not a payment import specification. Import IDs as text and preserve leading zeros. Amounts are USD. Keep bank details, tax identifiers, patient data, customer addresses, and confidential source content out of these files. Store evidence in an access-controlled system and use references here.

The example uses invented rates and agreements. Replace them with your approved terms, due dates, authority rules, and legal requirements. No formulas or external links execute in these CSVs. Calculate values in your controlled working copy and validate totals before relying on them.

## Read the quantities before totaling dollars

Each row is one approved compensation component for an agent, program, and period. Its source quantity may use a different unit from the payable quantity. Never add hours, events, tickets, and bonuses as if they were the same measure.

H1: 17.75 connected hours plus 0.75 separately verified eligible hours = 18.5 approved hours. At $24, the amount is $444. The source difference is explained, not deducted.

T1: 126 resolution events less six repeated events = 120 unique accepted tickets under this example's terms. At $2.50, the amount is $300. Q1 is a separate $30 bonus decision. Total approved is $774. All lines are unpaid in this example.

Retain underlying ticket IDs and timestamps in the source system. The summary row must link to the accepted unit list so duplicate detection works below the agent total. Keep source time zones and agreed cutoff definitions available. The sample period is inclusive; no cross-zone conversion is modeled.

A quality evaluation does not itself authorize a deduction. Record the contractual rule and authorized decision separately. Reopenings may be payable under other agreements; the example's six events are not a universal rule. Corrections after payment need a new authorized component linked to the original line, while preserving the historical amount.
