# Dispute worksheet instructions

Use one case per dispute with separately identifiable components. The example claims 12 inspections at $100 plus $80 mileage: $1,280. Initial approval is 11 inspections at $100 plus $80: $1,180. The disputed component is $100. Acceptance adds $100, making final approval $1,280.

Don't sum initial approval and final approval as separate liabilities. If $1,180 was already paid, the remaining approved adjustment is $100. The example leaves payment outcome open because approving an adjustment does not verify delivery.

Record the amount already paid, case receipt time, applicable due dates, decision authority, and next update time in the added columns or your case system. Blank amount-paid and date fields mean not recorded, not zero or no deadline. Verify them before release. The template does not set a universal deadline or permit withholding. Store sensitive evidence in restricted systems and put only references here. A legal interpretation belongs to the authorized adviser; a bank trace belongs to payment operations.
