# Worksheet instructions

These files contain a blank template and synthetic worked example. They are planning records, not a payment import specification. Import IDs as text and preserve leading zeros. Amounts are USD. Keep bank details, tax identifiers, patient data, customer addresses, and confidential source content out of these files. Store evidence in an access-controlled system and use references here.

The example uses invented rates and agreements. Replace them with your approved terms, due dates, authority rules, and legal requirements. No formulas or external links execute in these CSVs. Calculate values in your controlled working copy and validate totals before relying on them.

## Fee version and travel

Each row is one compensation component. F1 pays $65 for assignments accepted before October 15; F2 pays $70 for assignments accepted on or after October 15 in the example. This rule uses acceptance date, not service date or payment date. Actual agreements may use a different trigger.

Three F1 surveys total $195. Two F2 surveys total $140. Add a separately agreed $45 return visit and 84 approved miles at a contractual $0.60 per mile ($50.40) for $430.40. The mileage rate is illustrative and is not described as an IRS rate. TRIP-01 records the trip once; keep its allocation to assignments in the evidence record.

CORRECTION-01 has an unresolved fee and zero currently approved. A blank rate means the decision is unknown, not that it is a zero-fee obligation. Exclude pending rows from the approved total but keep them assigned for timely resolution. If a later decision authorizes $25, create that additional obligation and retain the original payment history. Do not resubmit $455.40 after the original $430.40 has been paid.

All approved lines are unpaid in this snapshot. Parent references explain relationships without treating new authorized work as a duplicate. Each line must retain its distinct earning-component identity.
